Lake County Golf Getaway Sweepstakes Rules and Regulations

1. NO PURCHASE NECESSARY. VOID WHERE PROHIBITED BY LAW. Please provide your name, address, city, state, zip code, phone number and email address to POSITIVELY CLEVELAND LAKE COUNTY GOLF GETAWAY SWEEPSTAKES by completing the online entry form. Entries must be transmitted by 11:59pm E.S.T. on Friday, July 31, 2009. By participating, each entrant accepts these rules.

2. Limit one entry per day.

3. Sweepstakes sponsors are not responsible for lost, late or misdirected entries. No phone calls, mail or facsimiles will be accepted regarding this sweepstakes.

4. Entries will not be acknowledged or returned.

5. All entries become property of Positively Cleveland (the Convention & Visitors Bureau of Greater Cleveland).

6. This sweepstakes is open to everyone 18 years old and older living in the United States except the employees of Positively Cleveland, the sponsors, their advertising agencies, suppliers and members of their immediate families. Email addresses may be provided to sweepstakes vendors for a one-time usage mailing.

7. Winner will be chosen on Monday, August 3, 2009. Winner will be notified by email or phone. Odds of winning depend on the number of eligible entries received.

8. Check participating venues for exact days and hours of operation. There may be blackout dates during holidays. Other restrictions may apply.

9. Only one grand prize will be awarded. Unclaimed prizes by August 14, 2009 will not be awarded. See below for prize package details.

10. No substitutions in prizes or cash will be allowed. Sponsors reserve the right to substitute
the prize with one of greater or equal value.

11. The winner and any persons accompanying the winner must comply with all instructions in the prize notification letter, complete an eligibility affidavit and release of liability, and return them to Positively Cleveland within ten days of the date on the prize notification letter. The winner must also agree to the use of his or her name and/or photograph for advertising and/or trade purposes where permitted by law.

12. Liability for any applicable federal, state or other taxes will be the sole responsibility of the individual winner.

13. This contest is subject to all federal, state and local laws and regulations.

Prizes can be mailed or picked up at The Higbee Building, Suite 100, 100 Public Square, Cleveland, Ohio 44113 between 9am and 5pm, Monday – Friday. Proof of ID required.
